Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 02/29/2024, between the hours of 10 am to 12 pm. There were 7 children present (16 points), ranging from toddler to preschool-age, with 1 caregiver supervising. The children were engaged in the following: snack, learning activities, music, song, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 7 children files, 1 staff file, and 2 household records were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard 8VAC20-800-60-B
Based on review, 2 of 7 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (02/29/2024), the following child did not list the action for the Provider to take for the possible exposure of the listed allergies - Child #7.
2. The following children did not have the policy name for the child's medical insurance - Child #6, and Child #7. The policy number was listed, but the policy name was not.
Plan of Correction: All missing information will be placed on file.
Standard 8VAC20-800-90-C
Based on review, 1 of 7 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations (for a child who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (02/29/2024), the current immunization page on file for Child #5 was dated - 01/04/2023.
2. On the date of inspection, Child #5 was under 2 years of age.
3. There was no religious exempt documentation on file for Child #5.
