(REPEAT VIOLATION) Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
1. In the Preschool A classroom, there was an unlocked upper cabinet with a spray bottle of disinfectant.
2. In the Prep-B classroom, there was an unlocked upper cabinet with a spray bottle of disinfectant and a spray bottle of sanitizer.
3. In the Prep A classroom, there was a wet jet mop with all-purpose cleaner attached and accessible to children.

Plan of Correction: Upon discovery, the upper cabinets in the Preschool A and Prep-B classrooms that contained cleaning materials were immediately locked to ensure the contents remain inaccessible to children. Although the cabinets were located above children's reach, they are now always kept locked in accordance with licensing requirements.
Additionally, the West Jet mop with the attached all-purpose cleaner located in the Prep A classroom was immediately removed from the classroom and discarded to eliminate the hazard.

The staff-to-child ratio for children ages birth to 16 months whenever children are in care is one staff for every 5 children.
From approximately 8:28am to 8:32am in the Infant 1 classroom, there was 1 staff observed with 6 children. The youngest child was age 5 months.

Plan of Correction: Upon identification of the issue, an additional staff member immediately provided support, and the required 1 :5 staff-to-child ratio was restored.
Staff were reminded of the required staff-to-child ratios and appropriate procedures for requesting assistance when ratios are approaching capacity. Leadership reinforced expectations regarding always maintaining ratios and ensuring appropriate coverage during classroom transitions. Center leadership will continue to monitor classroom ratios throughout the day to ensure compliance with licensing standards.
