Inspector Notes
A renewal inspection was conducted on 10/18/2023 between the hours of approximately 11:30 am and 1:35 pm. There were 80 children present, with 16 staff supervising. Children and staff were observed while eating lunch. Lunch today consisted of mashed potatoes, turkey and gravy, carrots, pear slices and milk.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
4Evidence :
1) On the date of inspection the record for Staff #2 (start date 08/28/2023) did not have available for review the following out-of-state background checks: Child Abuse and Neglect, Sex Offender Registry search, Criminal History name check. Staff #2 had declared in their sworn statement dated 08/29/2023 having lived out of the state of VA within the last 5 years.
Evidence:
1) The allergy lists available for review in four of the six rooms were dated 03/13/2023. Information in these lists were not current since children had transitioned to other rooms or had left the program. One of the six rooms did not have an allergy list available for review.
Evidence:
1) The medication authorization form for one emergency medication for Child #4 had an expiration date of September 2023.
Evidence:
1) The records for Child #1, Child #2 and Child #3 did not contain the work phone number of each parent who has custody.
