Inspector Notes

A renewal inspection was conducted on 10/18/2023 between the hours of approximately 11:30 am and 1:35 pm. There were 80 children present, with 16 staff supervising. Children and staff were observed while eating lunch. Lunch today consisted of mashed potatoes, turkey and gravy, carrots, pear slices and milk.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.035-B-4
Based on record review, the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the results of an out-of-state Sex Offender Registry search request and out-of-state Criminal History Name Check that were to be obtained prior to the first day of employment; and the results of an out-of-state Central Registry search request within 30 days of employment.

Evidence :
1) On the date of inspection the record for Staff #2 (start date 08/28/2023) did not have available for review the following out-of-state background checks: Child Abuse and Neglect, Sex Offender Registry search, Criminal History name check. Staff #2 had declared in their sworn statement dated 08/29/2023 having lived out of the state of VA within the last 5 years.
Plan of Correction: Some of these out-of-state background checks were obtained on 10/18/2023 and 10/20/2023. Others have been requested between 10/20/2023 and 10/24/2023.
Standard 8VAC20-780-40-M
Based on observation and document review, the center did not ensure to maintain a current written list of all children's allergies, sensitivities, and dietary restrictions and that it is accessible to all staff who work with children.
Evidence:
1) The allergy lists available for review in four of the six rooms were dated 03/13/2023. Information in these lists were not current since children had transitioned to other rooms or had left the program. One of the six rooms did not have an allergy list available for review.
Plan of Correction: Teachers have given up-to-date forms for children's allergies and each classroom has a designated spot for them.
Standard 8VAC20-780-510-P
Based on medication and documentation review, medication authorization form for one medication in the center was expired.
Evidence:
1) The medication authorization form for one emergency medication for Child #4 had an expiration date of September 2023.
Plan of Correction: Spoke with parents on 10/18/23 to update the medication form. Parents have sent the form to the pediatrician to be signed and completed.
Standard 8VAC20-780-60-A
Based on record review, children's records did not contain all required information.
Evidence:
1) The records for Child #1, Child #2 and Child #3 did not contain the work phone number of each parent who has custody.
Plan of Correction: Requested parent's work phone number. If parents didn't have a work number parents placed n/a on the form and/or added a cell phone number to the form.
