Inspector Notes

A monitoring inspection was conducted on 06/14/2024. There were a total of 72 children with 11 staff members present. Children were observed on the playground, engaged in circle time and centers, washing hands and during diapering procedures. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 8 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today in order for it to be included on the website. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard 8VAC20-780-270-A
Based on observation, the cdc did not keep areas and equipment of the center, inside and outside, maintained in a safe condition.
Evidence:
1. In the monkeys classroom there was a broken upper cabinet door hanging by the top hinge on one door.
2. In the pandas classroom there were 2 allegator toys that had a pull string attached approximately 2 ft in length accessible to children posing a strangulation hazard.
3. On the two year old playground there were 2 spoons with strings approximately 4 ft in length and 1 spoon with a string approximately 2 ft in length attaching them to the fence within reach of children posing a strangulation hazard.
4. On the preschool playground there were 5 colorful belts hanging from the fence that were approximately 3 1/2 ft in length within reach of children. There were 4 buckets with ropes attached approximately 1 1/2 to 2 ft in length within reach of children. There was a ribbon approximately 2 1/2 ft in length tied to a metal ball basket within reach of children. All posing a strangulation hazard.
Plan of Correction: 1. Submit work order to repair cabinet.
2. Cut pull string off or remove toy.
3. Remove strings and store spoons in basket.

Based on observation, the cdc did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the teddy bear classroom there was 1 cleaning spray on top of the mirror above the sink in the bathroom.
2. In the pandas classroom there was 1 cleaning spray and 1 cleaning wipes package in an unlocked upper cabinet. There was 1 cleaning spray in the bathroom on top of the shelf above the changing station. There were 2 cleaning sprays and 9 cleaning wipes in an unlocked upper cabinet above the toilet.
3. In the monkeys classroom there was 1 cleaning spray and 2 cleaning wipes in an upper unlocked classroom cabinet. There was 1 cleaning spray hanging from the upper bathroom cabinet and inside the unlocked cabinet there were 2 Lysol wipe packages and 1 cleaning spray.
Plan of Correction: Ensure all classrooms have locks to secure cleaning products in cabinets and review policy with all staff.
