Inspector Notes

An unannounced annual subsidy vendor inspection was conducted today from 12:15am until 1:00pm. There were 2 children directly supervised by the provider. The home, provider record, 1 child?s record, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating snack. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on provider statement, the vendor did not maintain, and keep at the family day home, written or electronic information for each enrolled child.
Evidence:
The record for Child 1 was not present in the family day home on the date of inspection. The provider stated that it was sent home with the parent.
Plan of Correction: The child has disenrolled. files will be maintained on site in the future.
