Inspector Notes

A monitoring inspection was conducted today between 8:45am and 10:15am. There were 6 children present with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed eating morning snack and dancing. A total of 5 child records and 2 caregiver records and two household members were reviewed. Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued. Standard 8VAC20-800-170-B
Based on review of caregiver records, the family day home did not obtain a tuberculosis (TB) screening from a caregiver, that was completed within the last 30 days before the first day of employment.
Evidence
Caregiver #1, (first day of employment 3/12/2024) had a TB screening dated 9/5/2023.
Plan of Correction: I didn't realize it needed to be within 30 days, going forward I will make sure the TB is within the time frame.
Standard 8VAC20-800-240-A
Based on observation, areas and furnishings of the family day home, inside and outside, were not maintained in a clean, safe condition.
Evidence
The side wall had a piece of wall that was ripped and a quarter size piece of the wall hanging.
Plan of Correction: Corrected during the inspection. The map came off the wall and ripped the wall. The map was put back up.
Standard 8VAC20-800-90-C
Based on review of 5 children's records, the family day home did not obtain documentation of additional immunizations every six months for a child under the age of two years.
Evidence
Child #1 (under the age of two) does not have documentation of updated immunizations every six months. Immunization record in file was dated. 4/2023.
Plan of Correction: I contacted the parent today to get the updated immunizations. I will review records to make sure all required documents are there.
