During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 03/04/2026 and completed on 03/24/2026. The on-site inspection began at 1:46 pm and ended at 3:47 pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and 4 staff. The inspector reviewed 8 children?s records and 5 staff records on-site and electronically on 03/13/2026. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/31/2026. A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The center did not request a search of the child abuse and neglect registry from one state for staff #1, who has been employed for 5 months.
Plan of Correction: Staff #1 was previously employed in our summer camp program. She left our employ to attend college. The state that she attended college in is a National Fingerprint File State which requires that the employee facilitate the background check. We obtained an out of state background check which showed the employee was eligible to work with children, however what was provided was not the correct form of the background check. The correct forms were completed and provided to the license inspector. They also show the employee is eligible to work with children. The offender check was requested on 03/12/2026 and the state background check was completed on 03/13/2026. The background checks were provided on 03/19/26.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

1. The screening for staff #1 was dated 1 year and 5 months prior to the date of employment.
2. The screening for staff #2 was dated 7 months prior to the date of employment.
Plan of Correction: Staff #1 and staff #2 were previously employed by our school. The TB tests noted by the inspector were completed at the time of original hire. TB tests are considered valid for two years. As rehires to the same entity, we have previously been counseled by licensing inspectors that new TB screenings were not needed for rehires so long as the TB screening is not out of date which these were not. We have now changed our practices to align with this new information from licensing. Both staff members completed the Virginia TB Screening and Risk Assessment Tool and Report of Tuberculosis Screening Child Day Programs. These were checked and signed off by our RN on staff with no concerns noted. This action brought their files into compliance.
