Inspector Notes

An unannounced monitoring inspection was conducted on 10/17/2023 from 12:20pm to 1:30pm. At the time of the inspection there were nine children in care, ages 8 months to 4 years old (24 points) with two caregivers. Records were reviewed for 5 children and 3 caregivers. Children were observed napping during the inspection. Emergency plans and emergency supplies were reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Based on record review and interview, it was determined the facility did not ensure that all applicants for employment, employees, applicants to serve as volunteers, and volunteers shall undergo a background check prior to employment or beginning to serve as a volunteer and every five years thereafter.
Evidence:
1.The provider did not have documentation of a repeat criminal history check for Household/Caregiver #1 and #2'. Both criminal history checks expired on 04/19/2023.

Standard 8VAC20-800-140-D
Based on record review and interviews, it was determined the provider did not ensure that the substitute provider records and signs the time of arrivals and departures on each day that the substitute works.
Evidence:
1. There was no record for the time of arrival of substitute #3. This substitute was providing care on the date of the inspection,
2. Provider #1 confirmed there was no record of the arrival of the substitute provider.

Standard 8VAC20-800-890
Based on observation and interviews, it was determined that the provider did not ensure that when food is brought from home the container will be clearly labeled in a way that identifies the owner.
Evidence:
1. There was a bottle of formula in the freezer that had no label identifying the owner.
2. The provider confirmed there was no name on the bottle.
