Inspector Notes

A, monitoring inspection was completed on Wednesday, April 27, 2023 at the center.
There were 25 children present with 6 staff supervising including administrator. The
inspector reviewed compliance in the areas of administration, physical plant, staffing and
supervision, programming, special care and emergencies and nutrition. A total of
4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff records and interview, the center did not ensure to obtain all staff background checks every five years thereafter.

Evidence: In Staff#3's record, the central registry findings was dated 06/07/17. Interview with staff confirmed findings available for review during the inspection.
Plan of Correction: The center's director will submit request to central registry finding for staff and include copy in file once completed.
