During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on April 25, 2025, and completed on June 6, 2025 with a local agency in response to a complaint received by the licensing office on April 24, 2025 relating to allegations of staffing and supervision. The inspector reviewed 1 child record and 2 staff records onsite. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 1, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

During an outdoor group activity on April 23, 2025, Child #1 (five years old) ran into oncoming traffic. An emergency traffic stop was executed by local agency representatives to remove the child from the street. The child was gone for an undetermined amount of time and staff were not aware that the child was missing until the group returned inside the classroom.
Plan of Correction: Proper disciplinary action was rendered to the staff member responsible for maintaining proper sight and sound of Child#1.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.

During an outdoor group activity on April 23, 2025, Child #1 (five years old) was without actual sight and sound supervision of staff for an unknown period of time when the child ran into oncoming traffic and was removed from the street by local agency representatives. The staff were not aware that the child was gone until they returned inside the classroom.
Plan of Correction: Our center has updated our policies and procedures and we have also reviewed additional training for all staff members.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The center?s supervision policy states that anytime our staff is supervising children, they will always ensure their care, protection and guidance. Our staff must also maintain sight and sound supervision for all students at all times except when using the restroom. On April 23, 2025, the center did not follow their own policy during an outdoor group activity, when Child #1 ran into oncoming traffic and was removed from the street by local agency representatives. The staff were unaware that the child was gone until the group returned inside the classroom.
Plan of Correction: Our center has updated our policy to require a minimum of 2 teachers to assist with the arrival and departure of all outdoor activities. Helping to ensure the proper sight and sound of our students.
