Inspector Notes

An allegation that ratios were not maintained on the morning of 05/08/2024 was received. A Complaint investigation was initiated today. Observation of all 7 occupied classrooms was made. Staff interviews, staff and child files and medication were reviewed. The preponderance of evidence supports a VALID finding regarding ratios in 3 out of 7 classrooms. Additional violations were found that were not part of the ratio allegation.

Standard 8VAC20-780-210-A
Based on staff record review, requirements for program leaders were not followed.
Evidence:
Staff #1, 4 and 5 are in program leader capacities. The requirements for program leads were not found.
Plan of Correction: We have gone through the files and will attach /submit lead qualifications. Additionally, if additional training is
needed, staff will make sure to complete these as soon as possible
Standard 8VAC20-780-245-J-3
Based on staff statements and review of medication, a child with an emergency medication was not within the presence of a staff member who meets the additional MAT training requirements.
Evidence:
1. An 8:06am arrival was documented in the electronic attendance record for Child #2.
2. Child #2 has an emergency medication which requires additional training.
3. There were no staff with current Auvi Q training onsite.
Plan of Correction: 6 staff members now have this training completed and an additional group of teachers will be completing this training as well.

SYSTEMIC
Based on observation of the center on 05/09/2024 a ratio of 1 staff for every 5 children were not maintained.
Evidence:
The Toddlers classroom had a total of 11 children, ages 16 months to 2 years with only 1 teacher at approximately 8:25am on 05/09/2024.
Plan of Correction: Staff schedules have been changed so that ratios will be maintained at all times.

SYSTEMIC
Based on staff interviews, a ratio of 1 staff for every 8 children were not maintained.
Evidence:
On 05/08/2024 at approximately 9am, the Preschool class had a total of 12 children with only 1 staff member.
Plan of Correction: Staff schedules have been changed so that ratios will be maintained at all times.

Based on observation of the 3 year old classroom, a ratio of 1 staff for every 10 children was not maintained.
Evidence:
On 05/09/2024 at approximately 8:40am, the 3 year old classroom had a total of 13 children with only 1 staff member.
Plan of Correction: Staff schedules have been changed so that ratios will be maintained at all times.

SYSTEMIC
Based on staff statements and observation of the center, children are assigned to a different age group on a routine basis.
Evidence:
1. Staff stated that in the mornings before 9am, children are routinely moved from the Infant 2 classroom to Toddlers classroom to maintain ratios.
2. Staff stated that in the mornings before 9am, one child is routinely moved from the Toddlers classroom to the Preschool classroom to maintain ratios.
Plan of Correction: Staff schedules have been changed so that ratios will be maintained at all times.
