Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 20, 2024. The on-site inspection began at 10:15 AM and ended at 12:30 PM. The inspector reviewed compliance in the areas listed above. There were 26 children present and 6 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: The record for Child #3 (date of enrollment 8/1/24) did not have documentation of the immunization required by the State Board of Health.
Plan of Correction: I will email the parents today that the immunization record and physical is due on or before August 30, 2024.
