An unannounced monitoring inspection was initiated and completed on February 27, 2025 in response to a self-report received by the licensing office on February 26, 2025, relating to administration, supervision of children and physical plant. The inspector reviewed two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Cosmetics, medications or other harmful agents shall not be stored in areas that are accessible to children. Staff #2 found a vape pen (model: Lost Mary MT15000 TURBO) sitting on the bathroom window sill in a classroom that is currently only used for the staff and children to use the restroom. Staff #2 notified the director, who immediately removed the vape pen. The director interviewed all staff to determine who the pen belonged to. Staff #1 confirmed the ownership of the vape pen.
Plan of Correction: Per the Director: "When I was told there was a vape pen in the bathroom, I immediately removed it. I determined by interviewing all my staff who the vape pen belonged to and that employee was terminated. We will review our policy with the staff that personal belongings must be inaccessible to children and smoking is prohibited on our property at our staff meeting on March 12, 2025."

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards. A vape pen was found on a window sill in a classroom bathroom at approximately 8:35am by an employee. The center requires personal items (cosmetics, medications, or other harmful agents) are stored in areas that are not accessible to children. The center prohibits smoking on the property.
Plan of Correction: The director stated the employee received orientation in October 2024 and again in January 2025. All their our policies were reviewed each time with the employee. After determining who the vape pen belonged to, the employee was terminated. We will review our policies and procedures on March 12, 2025 with all staff.
