During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/10/25.The on-site inspection began at 11:30am and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 10 children present and two caregivers; the point total was 30. The inspector reviewed five children?s records, three caregiver's records, and one household member's record [on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/28/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
The record for Child #1 (enrolled for over a month) did not contain documentation of a physical examination.
Plan of Correction: I made the parents aware and they were able to provide me with a physical.
Standard 8VAC20-800-110-B
Each report shall be signed by a licensed physician, the physician's designee, or an official of a local health department.
The immunization reports for Child #2, Child #3, and Child #4 did not contain a signature by a licensed physician, the physician's designee, or an official of a local health department., which is required.
Plan of Correction: I sent parents a message explaining to them that all physicals and shot records have to be signed by a physician.
Standard 8VAC20-800-120-B
Assistants' records shall contain the following: verification of age.
The record for Caregiver #1 (employed for two months) did not contain verification of age, which is required.
Plan of Correction: I made a copy of staff's driver's license and put it in her file.
Standard 8VAC20-800-60-B
Repeat Violation
A child's record shall contain all the required information.
The records for Child #1 (enrolled for one month) did not contain all the required information.
Plan of Correction: I have went through all the children's files to ensure all slots were filled out correctly.
Standard 8VAC20-800-880
When family day homes provide meals or snacks, the following shall apply: Family day homes shall follow the most recent, age-appropriate nutritional
requirements of a recognized authority such as the Child and Adult Care Food
Program of the United States Department of Agriculture (USDA).
The lunch served on the day of the inspection did not meet all nutritional requirements. The lunch did not have a vegetable.
Plan of Correction: I held a staff meeting/refreshing course explaining the importance of children having all their food components.
