Inspector Notes
A Subsidy Health and Safety inspection was conducted on January 11, 2023 to review health and safety requirements. On the date of inspection 2 children were in care. An inspection of the home, children's records, staff and household member records, emergency policies, and supplies was conducted Violations were found during the subsidy health and safety inspection requirements and are identified on the violation notice.

1.Child 2's record did not contain documentation that the child's age and identity have been verified or authorization for emergency medical treatment.
2. Child 3's record did not list the name, address, and phone number for a second parent or guardian; the name, address and phone number of wither parent's employer, or an address for a person to contact in the event of emergency.

There was no documentation to confirm a shelter-in-place drill had been conducted in 2022.
