Inspector Notes

An unannounced monitoring inspection was conducted on 12/14/2022. There were 15 children, ages 2 years-5 years, 4 staff members providing direct supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services. The children were observed during meal time, and teacher directed activities. The inspector discussed the following with the provider: carbon monoxide detector. A total of 5 children?s records, 5 staff records were reviewed. The inspector arrived for the inspection at 8:45 am and departed at 10:45 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation noticed issued to the provider.

Based on record review and interviews with staff, the center failed to ensure that children shall always be within actual sight and sound supervision of staff.
Evidence: Child 1 and Child 2 were observed in the room across from the kitchen receiving services from an outside agency. There was not a staff member present in the room. The children were not within sight and sound supervision of a staff member during this time. Staff 1 confirmed that the person providing service was not considered a staff person.
Plan of Correction: : Staff will obtain authorization to pick up from family .
