Inspector Notes

An unannounced renewal inspection was conducted on 10/10/2023. There were 18 children, ages 2 years - 5 years, 5 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies and nutrition. The children were observed during teacher directed activities, self-directed activities and mealtime. A total of 5 children?s records, 5 staff records and 5 agent/applicant records were reviewed. The inspector discussed the following with the provider: training and physical plant. The inspector arrived for the inspection at 9:45 am and departed at 12:15 pm.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and interview with staff, the center failed to ensure the injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.

Evidence: Injury prevention procedures were last updated January 2022. Staff 6 confirmed that was the most recent update.
