Inspector Notes
An unannounced monitoring inspection, resulting from a self-reported incident received on 04/19/2024, was conducted on 04/29/2024. The inspector reviewed video footage, 1 children?s record, 1 staff record and interviewed staff. The inspector arrived at approximately 11:50 am and departed at approximately 1:30 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on video footage, the center failed to ensure that actions towards children that are forbidden by the standards (physical punishment, striking a child, roughly handling, or shaking a child) were not used at the center.
Staff 1 was observed roughly handling Child 1. The video footage shows Staff 1 repeatedly picking up Child 1 off of the ground by one arm.
