Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/25/2026. The on-site inspection began at 10:00 a.m. and ended at 2:00 p.m. The inspector reviewed compliance in the areas listed above. There were 25 children present and 4 staff. The inspector reviewed 5 children?s records, 5 staff records, and 3 medications on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/03/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

A spray bottle of disinfectant, an aerosol can of disinfectant and two containers of disinfectant wipes were in an unlocked cabinet in the four-year-old classroom.
Plan of Correction: Staff will lock the cabinet.

Centers shall be in compliance with the center's own policies and procedures required by these standards.

The center has a medication policy to document all administered medication. Between 03/13/2026 and 03/24/2026, medication 3 was administered to Child 5, seven times without being documented on the medication log sheet.

Medication 1 was stored in an unlocked lunchbox, and Medications 2 and 3 were stored in an unlocked backpack. The lunch box and backpack were being carried by Staff 1.

Centers shall keep a record of medication given children, which shall include all required information.

Between 03/13/2026 and 03/24/2026, medication 3 was administered to Child 5, seven times. The required documentation did not have the child?s name, the type of medication, and any adverse reactions/medication errors.

At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff 2 was alone with a group of six children (age 4 years) and was not certified in CPR or first aid.
Plan of Correction: Staff was certified same day.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

During the inspection, there were 6 children present in the four-year-old room and 7 children were marked present on the attendance record. The attendance did not document departure as it occurs.
Plan of Correction: Attendance will be updated as children arrive and depart.
