A subsidy health and safety inspection (SHSI) completed on 4/17/2025. The inspection started at 9:50 am and concluded at 10:25 am. There were (3) children present and (1) caregivers; the point total was (5). The inspector reviewed (4) children?s records and (4) caregiver/household members records on-site.
