Inspector Notes

A subsidy health and safety (SHSI) inspection was initiated onsite on July 26, 2022 from 9:40 a.m.-11:20 a.m, and concluded on August 1, 2022. There were 37 children present with five staff supervising.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-550-2-a-2
Based on a review of staff records, the center failed to ensure that a child protective service central registry check had been completed and on file.

Evidence:
1. Staff #3's date of hire was 05/25/2022. There was no documentation in the record that the central registry check has been completed.
2. Administration verified that the check had not been completed.
Plan of Correction: The employee submitted the updated notarized form and it was uploaded.
Standard 8VAC20-790-570-2
Based on a review of injury reports and interview, the center failed to report a serious injury that required outside medical care within two business days of the injury.

Evidence:
1. An injury reported dated 07/20/2022 reported a child hit his head on the gym floor, and the parent transported him to the emergency room to be evaluated.
2. Staff #1 stated she had reported and sent the report to administration. Administration verified that the injury had not been reported to the department.
