Inspector Notes
A subsidy health and safety inspection was initiated on 10/13/2022 and concluded on 10/14/2022. There were 48 children present, ranging in ages from five years to twelve years, with seven staff. The Inspector reviewed five child?s records and seven staff?s records. The physical plant was observed and the provider was interviewed regarding policies and procedures.Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the vendor failed to have a completed search of central registry within 30 days of the beginning date of employment for three out of seven staff.
