During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on 4/7/2025 and completed onthe same date. The inspection started at 10:10 and concluded at 11:There were 5 children present and 1 caregiver; the point total was 12. The inspector reviewed 4 children?s records and 2 caregiver and household member records.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Standard 8VAC20-790-170-D
The vendor shall ensure that the family day home does not exceed the capacity of children cared for as allowed by law or regulation.
The vendor had 5 children in care at the time of the inspection. The 5th child had been in care since 9:10.
Plan of Correction: I am planning on disenrolling my part time children because they show up on the days they are not suppose to come. Today a child was dropped off that is usually in head start. I had his dad pick him up at 11. I will make sure I only have 4 children in care.
Standard 8VAC20-790-180-B
The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
The vendor has one child signed in, the other 4 children in care are not on the attendance sheets.
Plan of Correction: I ask the parents to write when they arrive on attendance sheet, but they don't do it. I will make sure I write the children on the attendance sheets from now on.
Standard 8VAC20-790-190-A
Vendors shall maintain, and keep at the family day home, written or electronic information for each enrolled child, which shall be made available to the department's representative upon request.
One child in care, child 4, is missing a written record. The child's older brother attends the program.
Plan of Correction: I gave the parent the package of paperwork but she did not return it yet. I will make sure I get paperwork back before I accept the child for care.

Have mattresses that fit snugly next to the crib so that no more than two fingers can be inserted between the mattress and the crib
The vendor has one infant in care, the crib used has a piece of very soft foam as the mattress, the Mattress is not snug and the mattress can easily be moved.
Plan of Correction: I have a crib Mattress in storage I will get it, I got it but thought it was so hard for a little baby.

Medication can be given to children with written authorization from the parent.
One child has an inhaler without a medication authorization.
Plan of Correction: I will return the medication, I have never given it to the child.

The caregiver administers only those drugs that were dispensed from a pharmacy and maintained in the original, labeled container
One medication on site is not in the original labeled container, the inhaler has the child's namae written in marker on it.
Plan of Correction: I will make sure I do not accept medication that is not in the original box.

Evacuation procedures shall be practiced at least monthly
The vendor has not recorded drills for the months of September- December 2025
