Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 01/24/2025. The inspection started at 10 am and concluded at 11:30 am. There were (3) children enrolled and (1) caregiver; the point total was (5). The inspector reviewed (3) children?s records and (1) caregiver record on-site on 01/24/2025.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-3
The Vendor did not obtain an updated copy of the results of a search of the central registry (CPS check) maintained pursuant to ? 63.2-1515 for any founded complaint of child abuse or neglect against them.

The most recent documentation obtained through the City of Alexandra indicated that the Vendor's last CPS check expired 13 months ago.
Plan of Correction: If a current CPS check cannot be found, then a new request of the central registry will be completed and sent. The finding will be shared with the Licensing Inspector.
Standard 8VAC20-790-250-F
Caregivers who work directly with children shall, annually attend the department's health and safety update course.

This course was not completed by the Vendor in 2024. This standard was cited previously during the annual subsidy inspection conducted in 2024 for the 2023 update course.
Plan of Correction: The update health and safety training will be completed before the end of the year.
