Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on October 21, 2024 and completed on October 21, 2024. The inspection started at 11:20am and concluded at 12:00pm. There was 1 child present and 1 caregiver; the point total was 3. The inspector reviewed 1 child?s record and 3 caregiver/household members records on site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The vendor will ensure self and household members over the age of 14 years will have an initial Central Registry search, and subsequent recheck every 5 years.

The vendor, and household member 1 did not have central registry results on hand.
Plan of Correction: We are waiting for the central registry checks to be returned.
Standard 8VAC20-790-210-A
A tuberculosis (TB) screening is required for the vendor and household members over the age of 18.

Household member 2 has not had an initial TB screening, which was due over one year ago.
Plan of Correction: He will have a TB screening done this week.
