Inspector Notes

An in-person annual subsidy inspection was initiated by licensing staff and concluded on 11/02/2022, between the hours of 9:30 am to 10:30 am. There were no children in care and no children living in the home under the age of 12 present with the vendor. The inspector reviewed two children?s records and two adult/household member records submitted by the vendor to ensure compliance with the health and safety regulations.

Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the Violation Notice issued to the vendor.

Standard 8VAC20-790-190-B
Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date- 09/05/2022) was missing proof of the child's identity and written authorization to administer prescription or nonprescription medications.
2. Child #2's record (Start date-09/05/2022) was missing proof of the child's identity, the sex of the child, name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency, names of persons other than the custodial parent who are authorized to pick up the child, and written authorization to administer prescription or nonprescription medications.
Plan of Correction: The caregiver will view the proof of identity for the children and document the record.
Standard 8VAC20-790-230-A
Based on record review and interview, the vendor did not obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
1. Child #1's record (Start date 09/05/2022) did not have documentation of immunizations.
2. Child #2's record (Start date- 09/05/2022) did not have documentation of immunizations.
3. Caregiver #1 confirmed that immunization records for Child #1 and Child #2 were not obtained before the children attended the family day home.
Plan of Correction: The vendor will get a copy of the children's immunization records from the parent and document the record.
