Inspector Notes

A Subsidy Health and Safety Inspection was conducted on 5/25/2023 from 10am through 10:55am. There were 3 children directly supervised by the vendor. During the inspection the home, activities and interactions with children, emergency supplies, procedures and drills were inspected. Three children's records, vendor information and household member information was reviewed. The children were observed resting, doing puzzles, practicing writing their names and playing basketball.

Information gathered determined violations with applicable code or requirements and are listed on the Violation Notice.

Based on record review and interview, the vendor did not maintain a written hard copy of attendance that documents the arrival and departure of each child as it occurs.
Evidence: The vendors attendance records documented the attendance of one child every day since March 30, 2023. The vendor has three children that attend the family day home on a daily basis. The vendor stated that one family does not sign their children in and out each day and that the vendor has not been signing the children in and out each day.
Plan of Correction: I will tell the parent that they need to fill out the attendance.

Based on record review and interview, the vendor did not maintain a record of the dates of practice drills.
Evidence: The most recent documented practice drill was dated February 2022. The vendor stated that they were confused by the practice drill log and didn't know that they needed to write down each drill.
Plan of Correction: I will start writing down each drill.
