A Subsidy Health and Safety Inspection was conducted on 5/16/2024 from 11:00am through 11:50am. There were 2 children directly supervised by the vendor. During the inspection the home, activities and interactions with children, emergency supplies, procedures and drills were inspected. Two children's records and vendor information was reviewed. Information gathered determined violations with applicable code or requirements and are listed on the Violation Notice.

Based on record review and interview, childrens records did not contain all required information.
Evidence: Child #2s record did not contain documentation that the vendor had viewed verification of the childs age and identity. The vendor confirmed that the family has not brough in the childs birth certificate.
