Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interview, and a virtual tour of the program.

A subsidy health and safety inspection (SHSI) was initiated on February 14, 2022 and concluded on February 15, 2022. There were 3 children present with the Vendor supervising. A code compliance inspection was also conducted on this date. The Vendor is current with First Aid/CPR and MAT certification but does not currently have children that may require medication. The Vendor has attendance recorded for the three children, has maintained information for each child in an organized manner, and has attended trainings this year.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program. Please call with questions 703-479-4679

Evidence:
1. Child 1 is under 2 years of age and does not have up-to-date immunization documentation. The last recorded date is over a year old.
