Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 4/8/2025. There were 2 children present and 1 caregiver; the point total was 8. The inspector reviewed 2 children?s records and 4 caregiver/household member(s) records on-site.

This inspection included document review, a tour of the home, interviews, observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The vendor shall ensure that all areas of the premises accessible to children are free of obvious injury hazards.

There was a balloon observed tied to the pack-n-play. The balloon has lost helium and was lying on the ground next to the pack-n-play. The ribbon tied to the pack-n-play and balloon is a strangulation hazard.
Plan of Correction: All balloon strings were removed during the inspection.

Shelter-in-place procedures shall be practiced twice a year.

There was one shelter-in-place drill practiced in 2024.
Plan of Correction: I already practiced one drill in 2025. I will practice another before the end of the year.
