Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, and a virtual tour of the program.

An annual subsidy health and safety inspection (SHSI) was initiated on September 27, 2021 and concluded on September 28, 2021. There were no children present with the Vendor as the children were in school.. A code compliance inspection was also conducted on this date. The Vendor has an attendance sheet, emergency drills documented, and an area for children to play.

Based on record review and interview, it was determined the Vendor does not have current certification in CPR.
Evidence:
1. CPR card shows it expired September 21, 2021 and the Vendor stated she has not attended a training.
