Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 06/25/2025. The inspection started at 10a.m. and concluded at 11:30a.m.. There were no children present and 1 caregiver the point total was 0. The inspector reviewed 2 children?s records and 1 caregiver and 1 household member records on-site.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
2employment, proof of identity, emergency medical treatment authorization and the provider's medication policy.
Child #1 and #2's information in their records did not include addresses for parent's employment and authorization for emergency medical treatment.
Child #1's record was did not include proof of the child's identity.
Caregivers who work directly with children shall annually attend at least 16 hours of training, to include the department's health and safety update course.
The provider did not obtain 16 hours of annual training and did not complete the annual health and safety update course.
