Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted on 7/24/25 from 10:15 am to 11:55 am. There was 1 child present and 1 caregiver; the point total was 3. The inspector reviewed 1 child's record and 1 caregiver's record on-site. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-A
The vendor must complete a repeat Central Registry Search (CRS), every five years.
The vendor's CRS has been expired for 6 months. A repeat search was not completed.
Plan of Correction: The caregiver will work on obtaining the CRS with the county.
Standard 8VAC20-790-210-B
Caregivers shall undergo TB screenings at least every two years from the date of the initial screening.
The caregiver did not obtain a new TB screening to this date since the last screening which was completed 2 years and 2 months ago.
Plan of Correction: The caregiver will update the TB screening.

Caregivers who work directly with children shall annually attend at least 16 hours of training, to include the department's health and safety update course.
The caregiver last completed the Virginia Health and Safety Update training on 5/31/23. The vendor was not able to provide any documentation of the required annual 16 hours of training between 2024-25.

Bottles shall not be propped or used while the child is in his designated sleeping location.
Child # 1 was observed laying in the crib. Child # 1's milk bottle was found on the mattress by the edge of the crib.
Plan of Correction: The milk bottle was removed during the inspection. Children will not be allowed to eat or drink in the crib.
