During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previousinspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on February 5, 2025. The inspection started at 9:05am and concluded at 10:30am. There were three children present and one caregiver; the point total was 10. The inspector reviewed three children's records and
three caregiver/household members' records on-site. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Standard 22.1-289.040-A
Repeat Violation. Background checks are required at the time of application and every five years thereafter for the applicant, and any other adult living in a family day home. There were no five-year repeat checks in the records. The central registry search for the applicant and two adult household members expired over nine months ago.
Plan of Correction: I thought my son had submitted the search. The caregiver contacted the son, and he submitted the three central registry request s during the inspection.
Standard 8VAC20-790-230-B
The vendor shall obtain documentation of additional immunizations once every six months for children less than the age of two years. Child #1 and Child #2, 19 months old, last documented immunization was over 12 months ago.
Plan of Correction: I will call the parents and tell them they must bring the updated immunization records this week. I will remind them I must have updated records every six months until they turn two years old.
Standard 8VAC20-790-390-E
The diapering surface shall be cleaned and sanitized after each use. The caregiver cleaned the diapering surface with soap and water but did not sanitized the surface.
Plan of Correction: I will buy a spray bottle today so I have the sanitizer available when I change diapers. I will clean the diapering surface with soap and water first and then I will sanitize the surface.
