Inspector Notes

An annual Subsidy Health and Safety Inspection (SHSI) inspection was initiated by licensing staff and concluded on 08/16/2023, between the hours of 12:30 pm to 1:30 pm. There were (4) children in care (10 points), with the Vendor. The inspector reviewed (4) children?s records and (5) adult household member records submitted by the vendor to ensure compliance with the health and safety regulations.

Information gathered during the inspection determined (1) area of non-compliance with applicable regulations or law. This violation was documented on the Violation Notice issued to the vendor.

Based on review, 3 of 4 children enrolled did not have documentation that each child had been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations.

Evidence -
1. On the date of inspection (08/16/2023), immunization documentation was not available for review for the following: Child #1 (start date - 08/01/23), Child #3 (start date - 06/01/23), and Child #4 (start date - 07/31/23).
2. The Vendor confirmed this finding on the date of inspection.
Plan of Correction: Documentation of an immunization page will be obtained and placed on file for Child #1, Child #3, and Child #4.
