Inspector Notes

An unannounced subsidy health and safety inspection was conducted on 01/12/2023. There was one child in attendance at the time of the inspection. Required background searches, training certificates and household member information was reviewed. A sample of requirements for daily attendance, child information/records, emergency preparedness planning and supplies were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor. Based on record review, the vendor did not obtain all information for children's records.

Evidence: Child #1's record did not contain proof of the child's identity.
Plan of Correction: The vendor contacted the child's parent during the inspection requesting that the parent bring the birth certificate by Monday.
Standard 8VAC20-790-210-B
Based on record review, caregivers did not undergo TB screenings at least every two years from the date of the initial screening.

Evidence: Caregiver #1's most recent TB screening is dated 09/25/2020.
Plan of Correction: I thought I had more time to get it done. I will schedule the appointment.
