Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on 4/19/21 and concluded on 5/7/21. A complaint was received by the department regarding allegations in the areas of care of the children and reporting requirements. The director was contacted by telephone to conduct the investigation. The inspector emailed the director/provider a list of documentation required to complete the investigation. The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued and can be found on the violation notice.

Based on interviews and a review of documentation, it was determined that the vendor did not inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.

Evidence:
1. The Licensing Inspector was provided information by the local agency on 4/2/21 that child #1 was injured at the facility on 3/24/21 and required outside medical treatment. However, the vendor did not inform the department's representative about the child's injury.
2. Staff #3 confirmed that the department's representative was not notified within two days of the injury.
3. A report about the injury was submitted after the Licensing Inspector contacted the facility about the injury.
Plan of Correction: The vendor responded: Moving forward, BBLC management will continue to self-report all children?s injuries immediately after being told that a child had to seek medical attention.
