During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on February 13, 2025. The on-site inspection began at 11:15am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 61 children present and 14 staff. The inspector reviewed 8 children?s records and 5 staff records on-site on February 13, 2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1 (age 21 months) has been at the center for 15 months and there was no updated immunization record to his file.
Plan of Correction: The center responded with the following: The center has a list of children that need annual updates and additional immunizations. The center will inform the parents that the information is required and will place the updates in the children?s file.
Standard 8VAC20-780-270-A
REPEAT VIOLATION
Areas and equipment of the center, inside and outside, shall be maintained in safe and operable condition. There are spots of peeling paint throughout the building on the baseboards, door handles, and wall trim. In the boy?s bathroom, there is an exposed rusted screw to the side of the commode.
Plan of Correction: The center responded with the following: The owner has discussed having a professional painter to come into the building to re-paint the walls of the classrooms. A work order will be put in for the screw to be covered.
Standard 8VAC20-780-500-A
Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. After lunch, Staff #1 changed a toddler child?s diaper and did not wash his hands after lunch or after the diaper change.
Plan of Correction: The center responded with the following: The center will have staff members re-trained on handwashing requirements.
Standard 8VAC20-780-500-B
Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Staff #1 was observed lifting the lid of the garbage to throw away a soiled diaper.
Plan of Correction: The center responded with the following: The center will have staff members re-trained on appropriately disposing diapers. A new foot-operated garbage will be placed in the classroom.
