Repeat Violation
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition. The following are areas of concern:
1. A table used by school age children is broken and cracked.
2. In the infant room:
a. There is a broken cabinet door in reach of children in care;
b. The half door has peeling paint and wood in reach of children in care;
c. The plastic mat used as flooring is peeling shedding plastic.

Plan of Correction: The center responded with the following: The table in the school age room will be thrown away and replaced with another table for children to use. For the infant room work orders have been placed for the cabinet to be fixed. The half door will be re-informed and repainted. The plastic mat will be thrown away, and a new area carpet has been ordered. Staff will continue to monitor equipment and the physical plant to ensure the safet of the children in care.
