Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced renewal inspection was initiated on March 26, 2021 and concluded on April 7, 2021. The provider was contacted by phone at 10:15 a.m. to initiate the inspection. Virtual inspection began at 10:45 a.m.. There were ten children in care ages 20 months through 4 years of age for a total of 25 points under the care and supervision of 2 staff. Children were observed having outdoor time. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed three children?s records, four staff records, one household member record and two pet records submitted by the facility to ensure documentation was complete. Emergency drills were reviewed.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on review of documentation, it was determined that the provider did not obtain documentation of sworn statement or affrimation by the first day of employment as required. Evidence: Staff #3, start date 6/1/2020, sworn statement was dated 6/7/2020.
Plan of Correction: Sworn statement will be completed by the first day of employment
Standard 22VAC40-191-60-C-2
Based on review of documentation, it was determined that the provider did not obtain documentation of central registry background check by the 30th day of employment. Evidence: Staff #3, start date 6/1/2020, central registry background check was dated 12/22/2020.
Plan of Correction: Central registry check will be completed by the 30th day of employment.
Standard 63.2-1720.1-B-2
Based on review of documentation, it was determined that the provider did not obtain documentation of fingerprint background check by the first day of employment or within 30 days of the household member turning 18 years old as required. Evidence: Staff #3, start date 6/1/2020, fingerprint check was dated 9/17/2020. Household Member #2 turned 18 years old on 2/21/2021 and there was no documentation of fingerprint results on file.
Plan of Correction: Fingerprint cehcks will be completed within 30 days of turning 18. *These procedures were impacted by COVID19.
