Inspector Notes

An unannounced, on-site annual SHSI inspection was initiated and completed on August 29, 2024, as a part of the licensure period. The on-site inspection began at 2:25pm and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 provider. The inspector reviewed 2 children?s records,1 provider record, and 1 household member record on-site. This inspection included document review, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review and interview, the provider did not ensure each child?s record included the required information.
Evidence:
Child #1 was missing address and phone number for custodial parent?s employment and proof of identity. Child #2 was missing address and phone number for emergency contact.
Plan of Correction: Call the parents to get the missing information.
