Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on today between 10:45am and 11:30am. There was 4 child present with 1 provider/vendor supervising. The physical plant, policies and procedures, emergency supplies and procedures, 2 children's records, 1 provider/vendor record and 1 household member record were reviewed. The exit interview was conducted with the provider/vendor. Areas of non-compliance are identified in this report. REPEAT VIOLATION
Based on review of children's records, all records did not include the required information.
Evidence
The record for child #1 did not contain the name, address and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: I will get the emergency contact information and make sure I have it before the child's first day.

Based on review of emergency procedures, an evacuation drill is not being practiced at least monthly.
Evidence
An emergency evacuation drill was not conducted in the month of November 2023.
Plan of Correction: I did a shelter in place drill that month I didn't realize I needed to do a fire drill too.. I will make sure to do a fire drill every month.
