Inspector Notes

A Subsidy Health and Safety Inspection was conducted on09/05/2022 from 1:00 am - 12:30 pm. There were 2 children (4 points) supervised by the vendor. During the inspection the home, activities and interactions, emergency supplies, procedures, drills were reviewed. Cleaning and sanitation routines and hand washing routines were also reviewed. Children's records, provider information and household member information was reviewed. Information gathered determined violations with applicable code or requirements and are listed on the violation notice. Based on a review of children's records, the vendor failed to obtain the telephone number for each custodial parent's place of employment.

Evidence:
Child #1 and #3's record did not contain documentation of the telephone number for each custodial parent's place of employment.

Based on a review of evacuation logs, the vendor has not practiced a practice evacuation monthly, a practice shelter-in-place drill two times per year or a lock down drill one time per year.

Evidence:
There is no documentation of an evacuation drill, a shelter-in-place drill or a lockdown drill after 03/2022.
Plan of Correction: I will re-start evacuation drills by September 20, 2022.
