Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted today between 10:15am and 11:30am. There were no children present with 1 provider/vendor supervising. The physical plant, policies and procedures, emergency supplies and procedures, 4 children's records, 1 provider/vendor record and 4 household member records were reviewed. The exit interview was conducted with the provider/vendor. Areas of non-compliance are identified in this report. Based on review of vendor general record keeping and records, a written hard copy of daily attendance that documents the arrival and departure of each child in care was not maintained.
Evidence
The last day of documentation of daily attendance that documented the arrival and departure of each child was March 9, 2023.
Plan of Correction: I will begin to immediately document the arrival and departure of all children as it occurs.
Standard 8VAC20-790-190-B
Based on review of child's records, all required information was not included.
Evidence
The record for child #1 was missing the work phone number for both parent's.
The record for child #2 was missing the work address for one parent.
Plan of Correction: I will get the missing addresses and phone numbers. I'll make sure to get all required information.

Based on review of evacuation procedures, drills were not practiced as required.
Evidence
Evacuation procedures were not practiced at least monthly;
Shelter in place procedures were not practiced twice a year
Lockdown procedures were not practiced at least annually.
Plan of Correction: I will document fire drills monthly, shelter in place drills two times a year and one lockdown drill a year.
