Inspector Notes

An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted On July 18, 2022. One child (4 points) was in care directly supervised by 1 provider/vendor. Ratio and supervision were in compliance. The physical plant, policies and procedures, emergency supplies and procedures, one child's record, one provider/vendor record, and two household member records were reviewed, Areas of non-compliance are identified in this report. The exit interview was conducted with the vendor/provider. Inspection conducted 10:00 am to 12:00 pm. Based on review of operational records, the vendor did not maintain a written hard copy of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence
The vendor did not maintain a written hard copy of daily attendance for the one child in care as they arrived and departed each day.
Plan of Correction: Corrected during the inspection, provider/vendor documented child #1's arrival time. I will begin immediately documenting the children's arrival/departure time.

Based on review of one child's record, the record did not include documentation of reviewing the child's proof of identity.
Evidence
The record for child #1 did not contain documentation of the vendor reviewing the child's proof of identity.
Plan of Correction: I will contact the parent today to ask for the original birth certificate. I will document that I reviewed it. I will not allow a child to start until I review the proof of birth.

Based on observation a non absorbent diapering surface was not available.
Evidence
The vendor confirmed a non absorbent diapering surface was not available.
Plan of Correction: I will buy a diaper changing pad and begin using that when changing a diaper. I will clean and sanitize the pad after each use.
