Inspector Notes

A subsidy health and safety inspection was conducted on August 15, 2024 between 10:20am and 11:00am. There was 1 child receiving care by the vendor. The physical plant, polices and procedures, emergency supplies and procedures, emergency drills, 1 child record, the vendors record, and 1 household member record was reviewed. An exit interview was conducted with the vendor. Areas of non-compliance can be found on the violation notice.

Based on observation, areas and equipment of the family day home, inside, were not maintained in a clean, safe, and operable condition.
Evidence:
1. The pack n' play used by child #1 (age: 15 months) had a hole in the mesh siding that was approximately 2in by 3in in size.
2. The Vendor acknowledged the hole in the pack n' play.
Plan of Correction: I plan to purchase a new pack n' play today.
