Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on 03/19/2025 and completed on 03/19/2025. The inspection started at 7:10AM and concluded at 7:35AM. There were no children present and 1 caregiver. There are currently no children enrolled. The inspector reviewed caregiver/household member records on-site.
This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/26/2025. A POC submitted after this date will not appear on the public website.

Caregivers who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training, to include the department's health and safety update course.

The health and safety update course was expired by 1 year.
Plan of Correction: Updated training was completed by the end of the day.
