Inspector Notes

A Subsidy Health and Safety inspection was conducted on 1/22/2024 to review supplemental health and safety requirements.
The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on review of attendance records and interview, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. The vendor stated Child #1 arrived at approximately 8:19am. The child was not listed on the attendance sheet for 1/22/2024 at approximately 9:16am when the inspection began.

Based on review of children's records, one child's information was not complete.
Evidence:
1. Child #1 (start date 1/04/2022) did not have a written agreement between parent and vendor.

Based on review of training and interview, one caregiver who works directly with children did not complete the department's health and safety update course annually.
Evidence:
1. The caregiver completed the health and safety update course on 2/20/2020 and had not competed it annually.

Based on observation, potentially poisonous substances such as cleaning agents and disinfectants were not stored in areas inaccessible to children.
Evidence:
1. In the bathroom that children use, a Clorox cleaner, shout cleaner, simple green and oxi-clean were inside an unlocked cabinet under the sink in the bathroom that children use.
