During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on October 8, 2025 and completed on October 8, 2025, as a part of the current licensure period. The on-site inspection began at 8:30am and ended at 9:30am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 14. The inspector reviewed 4 child files and 3 caregiver/household members files on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 15, 2025. A POC submitted after this date will not appear on the public website.
Violations
5The TB screening documentation for caregiver 2 was not signed by a medical professional.
Caregivers 1 and 2 did not have documentation of trainings. Caregiver 1 stated that the caregivers did not take any trainings in 2025.
The record for Child 4 was missing the employment address for both parent. The child was present during the inspection and had been in care for two months.
Caregiver 2 was observed changing a diaper and did not wash their hands afterwards.
Caregiver 2 did not wash child's hands after a diaper change.
