Inspector Notes

An annual Subsidy Health and Safety Inspection (SHSI) inspection was initiated by licensing staff and concluded on 07/24/2023, between the hours of 9 am to 10:30 am. There were 3 children enrolled in care. The inspector reviewed (3) children?s records and (2) adult/household member records submitted by the vendor to ensure compliance with the health and safety regulations.

Information gathered during the inspection determined that there was (1) area of non-compliance with an applicable regulations or law. This violation was documented on a Violation Notice issued to the vendor.

Based on review, current documentation that the department's health and safety update course was completed was not available for review.

Evidence -
1. On the date of inspection, 07/24/2023, a 2022 certificate of completion for the department's 3 hour annual health and safety course was not available for review.
2. The Provider stated that she did not complete this course last year.
Plan of Correction: This course will be taken and completed for 2023. A certificate of completion will be placed on file.
